This form is for Advanced Practice Trainees to complete to advise the Midlands Faculty for Advancing Practice of changes to their circumstances.  However, we recognised there may be instances where the Advanced Practice Lead / Training Hub Lead needs to complete this form on behalf of the trainee. 

Please refer to section 5 Trainee Advanced Practitioner Support of the A Guide to Implementing and Funding Advanced Practice in the Midlands  in conjunction with completing this form. 

The Midlands Faculty of Advanced Practice is required to keep up to date records of current NHS England funded Advanced Practice Trainees to enable it to better support and keep track of who is currently on the programme and requires continuation of funding.

It is therefore mandatory that we are informed about any changes to trainee’s personal and / or training situation by filling in and submitting this form as soon as possible and at least one month before any foreseen changes are to be introduced. Not completing the form can affect future NHS England funding allocated.

A change of circumstances refers to the following changes:

  1. Change of personal details (i.e. name, email address, phone number) 

  2. Change of university and / or course

  3. Change of co-ordinating educational supervisor 

  4. Change of employer

  5. A break from the advanced practice training programme (i.e. maternity leave, long term sickness, career break etc.)

  6. Withdrawal from advanced practice training programme

  7. other 

Once submitted, the Midlands Faculty of Advanced Practice will update the trainee’s training records and may need to discuss the changes with relevant parties (i.e. Employer, Advancing Practice Lead, Educational Supervisor, University, etc.) 

Your information will be securely stored by NHS England as the data controller. Personal data that we gather as part of this process will only be accessed, seen, and used by staff who need access to the personal data to fulfil their role. We will not share your personal data with any third-party organisations that are not involved in your training.
