Educators Referral

By submitting this referral you confirm that the trainee being referred accepts the following terms: 

-A case manager will be allocated to the trainee and as part of the process a case file (both paper and electronic) will be opened.

-The information within their PSWS case file is confidential and will only be accessed by members of the PSWS or named case workers.

-The PSWS will seek the trainee's permission before any information about them is shared with you as their educator. 

- Any information which could compromise the trainee's own or their patients’ safety cannot be kept confidential and will be shared at the discretion of the Associate Dean for Professional Support HEETV and Director of the PSWS.
