If possible, this form should be completed by the trainee and their Educational Supervisor, before the trainee is due to go on absence.

The return to work scheme is required for absences of 3 months or more but can be opted into for absences less than 3 months if trainee or Educational Supervisor feel it is necessary.

The information collected in this form will be securely held by HEE and will only be accessible to those directly involved in the Supported Return to Training; including the Professional Support Unit. Information may also be made available to the trainees employing Trusts / GP School Supported Return to Training Champion/administrator, Head of School, Training Programme Director, School Specialty Manager and Postgraduate Medical Education Centre. Data will not be shared wider without your explicit consent.
