Welcome to the SuppoRTT CPD Funding Request Form

This request form is to be used if you are wanting to attend a course which is not directly organised or overseen by the SuppoRTT Team, as listed on the SuppoRTT Webpage. This form is to be completed with the Trainee and Appropriate Educator/Supervisor*.

Completion of this form does not guarantee eligibility for funding. This is an application for funding approval.

Please note the following:

  • Retrospective applications for funding may be considered on a case by case basis whilst this new process is embedding, however this is not the norm and we reserve the right to deny retrospective applications.
  • To obtain funding for courses the trainee must have completed an Initial Return to Training Meeting Form prior to funding request. The form can be found on the SuppoRTT Webpage under the section  'Guidance and Forms'
  • SuppoRTT CPD Funding is not to be used for courses which are deemed to be career enhancing or a mandatory part of training. Applications of this nature will be declined.

*For the purpose of this document, ‘Appropriate Educator/Supervisor’ can be categorised but not limited to; Educational Supervisors (ES), College Tutors, Clinical Supervisors (CS), Foundation Programme Directors (FPD), Training Programme Directors (TPD), Head of Schools (HoS), Directors of Medical Education (DME), SuppoRTT Champion.

The information collected in this form will be securely held by HEE and will only be accessible to those directly involved in the Supported Return to Training.  Information may also be made available to the trainees employing Trusts i.e HR and Medical Education / Supported Return To Training Champion / administrator, Head of School, Training Programme Director.  Data will not be shared wider without your explicit consent.
